PELINI: Garcinia camboga Epatite Fulminante

In merito all'alert del Ministero della Salute sugli integratori contenenti Garcinia camboga si sa da tempo che può provocare effetti tossici all'livello Epatico. 

 


La Garcinia Cambogia è un prodotto a base di erbe derivato dal frutto dell'albero di tamarindo Malabar (chiamato anche Garcinia gummi-gutta) originario dell'India, del Nepal e dello Sri Lanka. La scorza essiccata e affumicata del frutto della Garcinia cambogia è comunemente usata come conservante alimentare e, per il suo sapore aspro e aspro, come aromatizzante e spezia, soprattutto nei curry di pesce. La scorza del frutto è stata utilizzata anche nella medicina ayurvedica per curare disturbi gastrointestinali e reumatismi. Più recentemente, la Garcinia Cambogia è stata ritenuta un soppressore dell'appetito efficace nell'indurre la perdita di peso. I componenti chimici degli estratti di Garcinia includono xantoni, benzofenoni, amminoacidi e acidi organici, soprattutto l'acido idrossicitrico (HCA), l'ingrediente sospettato responsabile della sua attività antinfiammatoria e soppressiva dell'appetito.Il frutto della Garcinia contiene dal 10% al 30% di HCA in peso e gli estratti possono contenere tra il 20% e il 60% di acido tricarbossilico. Epatotossicità Ci sono state almeno una dozzina di segnalazioni di danno epatico indotto da Garcinia in individui che assumono integratori alimentari multi-ingredienti (MIDS) che contengono Garcinia cambogia. In alcuni casi, nei prodotti MIDS erano presenti altre epatotossine note o sospette (come con i prodotti Exilis, Herbalife e Hydroxycut). Più recentemente, sono stati identificati casi di danno epatico acuto in persone che assumevano un prodotto etichettato come contenente la sola Garcinia Cambogia. La frequenza delle reazioni avverse epatiche alla Garcinia cambogia non è nota, ma è probabilmente non comune e in meno di 1:10.000 persone. I pazienti in genere presentano affaticamento, nausea, aumenti dei livelli sierici di aminotransferasi e ittero da 1 a 4 settimane dopo l'inizio del prodotto, sebbene in alcuni casi la latenza all'insorgenza sia stata più lunga (da 3 a 12 mesi).Il pattern degli aumenti degli enzimi è epatocellulare e le caratteristiche immunitarie non sono comuni. Punteggio di verosimiglianza: B (probabile causa rara di danno epatico clinicamente evidente). Meccanismo di lesione La causa dell'epatotossicità da Garcinia cambogia non è chiara. Studi in vitro suggeriscono che l'HCA può essere tossico per il fegato a dosi elevate, ma i rari casi di danno epatico acuto che si verificano con la Garcinia Cambogia suggeriscono una forma idiosincratica di danno. Risultato e gestione La gravità del danno epatico varia da aumenti enzimatici transitori e moderati all'epatite acuta sintomatica fino all'insufficienza epatica acuta. Nella maggior parte dei casi, il danno epatico scompare entro 1-3 mesi dall'interruzione del prodotto a base di erbe, ma se si sviluppa un'epatite fulminante, può essere necessario un trapianto di fegato.
